After years of nursing and supporting people with diverse and changing needs, I kept seeing the same thing. A participant would be discharged with a solid plan, a folder of instructions and the best intentions from everyone involved — and by day three at home it had quietly fallen apart. Not because the plan was wrong. Because nobody was standing in the kitchen at 6am when the routine needed to actually happen.

What started as helping a few individuals became something bigger. I built Together Care Collective to do the part of the job that's hardest to staff and easiest to get wrong: the transition, and everything that has to hold together afterwards.

Today we're a team of experienced support workers backed by clinical oversight, working across Deception Bay and the Moreton Bay region. We take the referrals that need a bit more thought, we answer quickly either way, and when we say yes, we turn up. Together Care Collective is a reflection of that journey — a team united by compassion, driven by quality, and grounded in the belief that we're stronger when we care together.

How we work

We answer quickly, either way

Same-day contact on every referral, with capacity confirmed inside 48 hours. If we don't have the right team available, we'll tell you straight away rather than leave you waiting.

Clinical oversight behind the scenes

Complex supports are planned and signed off with nursing input, and our team is trained on each person's own routines rather than given a general induction and a run sheet.

The same faces

Familiarity matters more in complex support than almost anything else. We build small, steady teams around each person, so nobody has to explain their morning routine to someone new.

Families in the loop

Where someone wants their family involved, we keep them genuinely informed with what changed, what we noticed, what we're adjusting.
