How we work with each of you

A discharge that fails in week one costs everyone — most of all the participant. Our involvement starts on the ward, not on the doorstep.
We attend before discharge. Sandie or a senior staff member comes to the ward to meet the participant, read the notes properly and speak with the treating team.
We check the home, not just the plan. Access, bathroom, doorway widths, bed height, equipment delivery status. Most failed discharges we've seen came down to something physical nobody had eyes on.
Staff are briefed before day one. Transfer technique, medication routine, communication, known triggers — walked through in person, not emailed.
We hold the first 72 hours. Senior staff on the first shifts, with a direct line to Sandie for the whole window.
You get a capacity answer inside 48 hours, and a provider who has already been to the house before the participant gets there.
Outdoor sport & recreation — informal games and active outings that build fitness, coordination and comfort in group settings
Mentor & peer support — regular one-to-one time with a mentor, building trust, communication and a sense of being understood
Community outings & local adventures — markets, parks, cafés and events, building confidence navigating the community independently
Participants become more comfortable starting a conversation, trying a new place, and taking part in community life without needing to be prompted.

Months of rehab gains can unravel in a fortnight at home if the daily routine doesn't carry them forward. Our job is to make the home the place where the work continues.
Continuity of technique. We take handover from the physio and OT on transfers, mobility and equipment use, and our staff do it the same way.
Therapy homework actually happens. Prescribed exercises and routines built into the support roster rather than left on a printout on the fridge.
Graded independence. Support reduces as capability returns, and we flag it at review instead of quietly holding the hours.
We report back. Written notes on function, fatigue and setbacks that your team can use.
The participant keeps what they gained in rehab, and you get visibility on how it's holding up at home.
Neurodivergent-inclusive programs — flexible structure, sensory-aware environments, and clear, predictable communication about what's happening and when
Parent hub — coffee, chat & community — an informal space for parents and carers to connect, compare notes and get support from people in the same position
